Assertive Community Treatment (ACT)  

Assertive Community Treatment (ACT) is an evidence-based model of case management for adults that provides comprehensive self-contained services including behavioral health and substance abuse treatment, health care coordination, criminal justice support and other needed services. Each of our 3 ACT teams works closely with individuals and families to reduce dependence on more restrictive settings and enhance their quality of life in the community. Our ACT teams tailor treatment to the individual and work on goals that have been identified by the individual. The provision of ACT services is ongoing and based on the acceptance and engagement in treatment. Our ACT Teams are multidisciplinary, providing psychiatric, nursing, clinical, forensic, vocational and benefits expertise to support individuals in their communities and support participant driven recovery.
